Irwin Army Community Hospital at Fort Riley issued a news release on Wednesday that said
all local active duty service family members who are currently receiving their healthcare by a civilian medical provider in the local community, and who reside within a 30-minute drive of that Hospital, should have received a letter stating they will be reassigned to one of IACH’s primary care providers with an effective date of June 15, 2014.
IACH’s primary care team provides a full range of comprehensive, quality medical services delivered in a Patient-Centered Medical Home environment.
Prior to June 15, beneficiaries will receive a letter with their new Primary Care Manager and their assigned Patient Centered Medical Home Clinic.
Also starting June 15, medical appointments can be scheduled by calling 785-239-DOCS.
If military families wish to remain with their current Primary Care Manager they can go to: https://iach.amedd.army.mil/, print a waiver request for consideration and submit the waiver by June 15 to the following address: IACH, Clinical Operations and Support Division, ATTN: Cindy Salee, 600 Caisson Hill Road, Fort Riley, Kansas 66442-5037.
